LSC ICS Proxy ordering of medication in Care Homes

Electronic ordering of medication in care homes offers a more efficient and safer process for appropriate medicines supply for care home residents.  The solution is not complicated but does require the completion of the following paperwork.

• Data sharing agreement between the care home and GP practice
• Patient consent form to allow electronic ordering
• Proxy ordering request form for care home staff

The paperwork has been reviewed and agreed by Healthier Lancashire and South Cumbria Proxy ordering working group as part of the Integrating NHS Pharmacy and Medicines Optimisation (IPMO) programme. The process is outlined below. Associated links and forms can also be found below.
